Documentations:
1. Assemble ALL of the documents below and go to your local Acceptance
Agent (usually USPS office has a passport acceptance office)
- A certified official United States Birth Certificate (Texas and
California “Abstract” birth certificates not accepted) or
naturalization certificate
- A copy of valid driver's license or State identification card
- Two Passport Photos
- Completed and signed application (the application must be
completed electronically at https://pptform2.state.gov/FraudAbuseNotice.aspx?Submit2=COMPLETE+FORM+DS-11+ONLINE
and printed out)**

1. Agree
to the privacy disclaimer. On the next page, click on "Apply Online"
2. Enter
all
information
requested
from the State Department
3. On the summary of fee page,
check only the $60 expedited fee and passport book fee and click submit
4. Click "Create Form" and
print out the form that will open as a PDF. Sign it and send it
along with your documents.
**As of
03/07/2011, the U.S. Department of State will no longer be accepting
the old application. Please download the new application and fill it
out online.
- Travel Itinerary (If you do not have travel itinerary yet, please
leave the trip plan section in blank on the DS-11 form)
- A check or money order to “US Department of State” for the amount of
$170.00. Write your date of birth on the government payment. A fee of
$25.00 is paid to the agent and may be paid in cash.
2. Tell the Acceptance Agent you intend to use “HAND-CARRY SERVICES”
3. The Acceptance Agent will take all documents, sign them and seal
them in a "security envelope"
4. Send us the sealed envelope with two Letters of Authorization (Download

Letter)
