A. New Passport:
Documentations:
1. Assemble ALL of the documents below and go to your local Acceptance Agent (usually USPS office has a passport acceptance office)
- A certified official United States Birth Certificate (Texas and California “Abstract” birth certificates not accepted)
- A copy of valid driver's license or State identification card
- Two Passport Photos
- Completed and signed application form
DS-11
- Travel Itinerary (If you do not have travel itinerary yet, please leave the trip plan section in blank on the DS-11 form)
- A check or money order to “US Department of State” for the amount of $170.00. Write your date of birth on the government payment.
2. Tell the Acceptance Agent you intend to use “HAND-CARRY SERVICES”
3. The Acceptance Agent will take all documents, sign them and seal them in a "security envelope"
4. Send us the sealed envelope with two Letters of Authorization (Download Sample Letter)
B. Renew Passport:
Documentations:
1. Your most recent
passport 2. A copy of valid driver's license or
State identification card 3. Two identical passport
photographs
4. The U.S. State Department requires applicants to electronically fill in
the application form (DS-82).
Fill them out and then print all of the
completed forms. 5. Two Authorization Letters For USCCC To Process

6. A Travel Itinerary
C. Adding Extra Pages:

1. Your most recent passport

form (DS-4085). Fill them out and then print all of the
